Long Term Care Program
Our Long Term Care Program goal, for chronically ill or disabled people, is to maintain your highest level of ability or health and to help you learn how to live with your illness or disability.
Our Long Term Care Program is an alternative to nursing home placement for the frail, disabled or chronically ill.
If you are eligible for this program, you will be able to remain at home and receive waivered services. These services may include:
- Audiology
- Home delivered meals
- Life line (personal emergency response system)
- Nursing care
- Personal care services
- Social or medical day care
- Therapy
