For over 20 years, Madison County’s Home Health Care Agency has been providing our community with skilled healthcare and personalized services. Our services provide a means for our patients to remain at home during illness, injury or disability.
Our Home Health Care is the only Certified Home Health Agency and Long Term Home Health Care Program in Madison County.

Our Home Care services provide our patients with appropriate health care services and skills to safely manage their illness in the comfort of their home.
We provide personalized treatment plans in the home for individuals of all ages, customized to meet a patient’s needs and short term or long term goals, based on physician’s orders. Our Short Term Care and Long Term Care programs and services are coordinated and personalized by a Public Health Nurse working together with the patient, caregiver, and physician.
Our new Telemedicine Program will provide patient’s with advanced healthcare monitoring equipment that will provide patient’s with more ability than ever before to participate in their own plan of care.
The types and amount of services provided may change depending on an individual’s needs and depending on the program enrolled (short term care or long term care program).
Our ultimate goal is to provide an optimal level of well being for our patients in a caring and safe home setting.

How Can I Get Home Health Care Services?
A referral must be made from sources including physicians or hospital discharge planners, or patients may search for visiting nurse providers in the phone book.
To be eligible, you:
- Need skilled nursing care or therapy
- Can safely be cared for at home
- Are a resident of Madison County
